For most of the Cold War, the Nuclear Suitcase accompanying the Secretary of Defence or President of the United States was just for show.
The suitcase was supposed to issue launch codes to ICBM crews, but the launch codes were all set to OOOOOOOO. Any silo based crew could have launched their ICBMs.
